What is Palliative Care?
Palliative care is person and family-centred care provided for a person with an active, progressive, advanced disease, who has little or no prospect of cure and who is expected to die, and for whom the primary goal is to optimise the quality of life.

This case study provides insight into the journey of a client with life-limiting illnesses, who opted to receive palliative care within the familiar surroundings and comfort of their own home.
The client's primary symptoms included severe pain, nausea, fatigue, and anxiety.
Through collaboration with our client, their family and following assessment our nursing team developed an individualised care plan, arranged equipment delivery (such as hospital bed, recliner chair and home oxygen) and managed transition home following discharge from hospital.
At home, pain management, symptom control, psychosocial support and collaboration with interdisciplinary teams were central to the nursing interventions.
Through a holistic palliative care approach, the client experienced improved symptom control and quality of life, ensuring a dignified end-of-life journey.
